We take time to understand what’s going on and talk things through properly. Support Coordination plays an ongoing role and works in tandem with our other services.
Support Coordination involves helping people make sense of their NDIS plan, organise services, and deal with problems as they arise. The same support coordinator stays involved over time, with regular contact by email, phone or face-to-face - according to your preferences.
Recovery Coaching supports people with psychosocial disability to build stability, routines, and confidence in everyday life. Support is paced carefully and adapts as mental health needs change.
Activities of Daily Living includes help with everyday tasks such as personal routines, household tasks, appointments, and organisation. The focus is on building skills and independence without rushing.
Community Participation supports people to take part in activities outside the home that feel meaningful and comfortable. Activities are chosen based on interests, not to fill time.
This service supports people through change, including plan reviews, life transitions, or times when needs increase. Support is provided before, during, and after changes so things feel more manageable.
Supported Independent Living supports people living in shared or individual homes with daily routines and ongoing needs. Support may be provided in a current home or in a home explored and set up together.
Individual and family therapy supports emotional wellbeing, relationships, and communication. Sessions are delivered by experienced clinicians and may involve family members where appropriate.
24/7 Complex Care supports people with high or complex needs who require consistent, structured assistance. Care is provided day and night, with attention to safety, routine, and continuity.

We work in a very hands-on way. We check in often, keep conversations going, and make sure people don’t feel left to manage things on their own once support is in place. With HHWSS, there's continuity as life changes and needs evolve.

Getting started usually begins with our referral form, which gives us some basic context about your situation and what you’re looking for. From there, we make contact to talk things through properly. This first conversation isn’t about locking anything in — it’s a chance to understand what’s happening, what support is already in place, and whether HHWSS is the right fit.
After reviewing the referral, we’ll get in touch to have a proper conversation. This may involve clarifying your NDIS plan, talking through current supports, and understanding what’s working and what isn’t. We take the time to answer questions and explain next steps clearly, rather than rushing into changes or decisions.
Needs change often, and that’s expected. As things shift — whether gradually or suddenly — we stay involved and talk through what needs adjusting. Changes are made with discussion and explanation, so you understand what’s happening and why, rather than feeling like things are being changed around you.
Yes. You can work with other providers at any time. Some people choose to keep certain supports separate, while others prefer to have things more connected. We respect those choices and focus on working alongside whatever mix of services feels right for you.
